CASE MANAGER

Three teams of case managers provide in-vivo and hospital based support for consumers who have serious and persistent mental illnesses. Services include assessment, psychoeducation and support, crisis intervention, systems advocacy. Direct assistance in obtaining basic necessities such as housing and entitlements, assistance with daily living activities, symptom assessment, management and supportive counseling, and substance abuse education and intervention. All consumers discharged from Trenton Psychiatric Hospital are served by ICM for 12 months. Second priority is given to consumers discharged from a short-term care facility. Community referrals are also accepted. This program is provided in collaboration with AAMH. Hours: M-F 9-5. Early AM, evening and weekend hours as needed. 24 hour/7day week emergency coverage.

JOB REQUIREMENTS:
• Bachelor’s degree in social work or related field plus minimum of two years experience in mental health working with adults with severe and persistent mental illness.
• Use of personal vehicle may be necessary in carrying out job responsibilities.
• Valid driver’s license required.

JOB RESPONSIBILITIES:
• Provides case management services including outreach, crisis intervention, and other interventions, as needed.
