RESIDENTIAL INTENSIVE SUPPORT TEAM
HOUSING COUNSELOR
The Residential Intensive Support Team assists adults
who are currently hospitalized at Trenton Psychiatric Hospital, and
need intensive support to reside independently. Services include
helping consumer to identify an apartment, manage her/his illness,
obtain mental health, medical and other needed social services,
assistance with employment, and other support which will assist
consumer in recovering from mental illness and reaching personal
goals. Support is provided 7 days per week, with 24 hour on-call
availability. RIST has recently been awarded new funding and is
expanding staffing.
JOB REQUIREMENTS:
Bachelor's degree from an accredited college or university with a
major in a mental health, health or other appropriate human services
related discipline; OR license as a registered nurse
Hours include evenings, weekends and 24 hour on-call (in person
response as needed).
Use of personal vehicle may be necessary in carrying out job
responsibilities.
Valid drivers license required
JOB RESPONSIBILITIES:
Performs outreach, engagement and assessment to enroll
hospitalized consumers in program services.
Assists clients in selecting and furnishing apartment and in
completing necessary rental and subsidy applications.
Uses knowledge of Recovery and Wellness, Motivational
Interviewing, Integrated Dual Disorder Treatment and other
evidence-based practices to assist clients to develop skills in 1)
independent living; 2) family and social supports; 3) vocational
preparation and readiness; 4) commitment to mental health treatment
and medication adherence and 5) commitment to sobriety.
Links clients to necessary services including mental health
services, rehabilitation, vocational and employment services, health
and dental services, income support and benefits, and substance
abuse treatment.
Assist consumers in problem solving.
Collaborates with consumers, family members and other providers to
develop services plans which identify needs, strengths and
strategies for meeting needs of individual consumers.
Provides crisis intervention and stabilization services as needed;
accessing the Screening Center as needed in resolving the crisis.
Prepares clinical, statistical, billing and personnel
documentation thoroughly and on a timely basis.
