GTBHC PROGRAMS AND SERVICES

 

We provide a broad array of mental health and addictions services to youth and adult residents of Mercer County and their families. Services include intensive day treatment; crisis-oriented outreach and in-home services; outpatient services; and a variety of services for people with special needs. Emergency on-call services are available 24 hours a day 7 days per week for those at the highest risk of crisis.

Outpatient services include individual, family and group counseling. Psychiatric evaluations and medication management are provided for clients who are also receiving counseling through the outpatient program. Services are available in English, Spanish and American Sign Language.

Partial care program for youth 12-17 with emotional/behavioral problems including co-occurring use of drugs and alcohol. The program provides up to 5 hours of group-based services daily. Other services include case management, individual counseling and support, addiction treatment including random drug and alcohol screening, skill development in activities of daily living, relationships, anger management and other essential skills, psychiatric evaluations and medication monitoring, and transportation.

Hours scheduled based on family availability including evenings and weekends.

Home-Based Services provides intensive in-community services for families referred by the CMO, CSA, or Youth Case Management. Services include assessment, individual and family treatment, family education and skill development which are provided by masters level or clinically licensed staff.

School-Based Services

The School-Based Program consists of three components in designated Trenton Schools:
1) ACTS serves children in grades 1-8 whose educational advancement is compromised by behavioral and/or emotional issues.
2) Truancy Outreach Services focuses on truant youth in grades 4-7.
3) Early intervention services are provided for children K-1. Services include brief contacts and interventions aimed at crisis stabilization, observation and assessment, individual play therapy, family therapy, group therapy, academic support, teacher consultation and support, linkage to other services and case management.

Children's Futures Behavioral Health

Hours scheduled based on family availability including evenings.

The Children’s Futures Behavioral Health Team works with Parent-Child Centers, Health Start Clinics, and other behavioral health care services to ensure that pregnant and parenting women receive necessary services for depression and mental health, tobacco cessation, drugs, alcohol, and domestic violence. Services include behavioral health assessments and linkage to services; in-home individual counseling, education and support groups at the Parent Child Development Centers, and consultation to Family Child Development Teams. Women served must reside in Trenton.

YCM is targeted to youth who have emotional and behavioral problems and need a moderate level of case management. Services include linkage with mental health, medical, financial, housing and educational services, family education and support, service coordination and advocacy, and crisis intervention assistance.

Partial care is a program for adults with a serious and persistent mental illness. Addiction services are integrated into the program for individuals with a co-occurring substance abuse disorder. Services include group counseling, work units, vocational training, addiction services, random drug and alcohol screening, psychiatric evaluations, medication management, transportation, and lunch.

Support for families who have a member with a serious and persistent mental illness. Services include outreach, consultation, psycho education groups and workshops, family counseling and support, advocacy. This project is provided in collaboration with NAMI Mercer New Jersey. New component geared to families with a member in jail.

Homeless Outreach Project

Various sites and hours.
Staff provides linkage to mental health services, medical services, entitlements and housing. Staff also provide daily support at the homeless drop-in center located at the Salvation Army. Regular outreach visits are also made to the Trenton Area Soup Kitchen, Rescue Mission, Food Court and local parks to assess and engage persons who are homeless and mentally ill. In addition, case management services, nursing assessments, psychiatric evaluations and medication management services are provided weekly at the Rescue Mission.

Senior Well Being Project

The Senior Well Being Project provides education to seniors and those who work with them on a variety of topics related to mental health. Through community outreach and staff training, the project provides consultation and information about mental health and related resources for seniors. The project offers a series of interactive presentations/groups to seniors on topics related to mental health. Groups can be provided at any location where seniors congregate. The project also provides support to caregivers (persons caring for seniors age 60 or older and seniors caring for children 18 or younger). Services include workshops, assessment of caregivers mental health needs, short-term counseling, and groups for grandparents raising children.

Integrated Case Management (ICM)

Three teams of case managers provide in-vivo and hospital based support for consumers who have serious and persistent mental illnesses. Services include assessment, psycho-education and support, crisis intervention, systems advocacy. Direct assistance in obtaining basic necessities such as housing and entitlements, assistance with daily living activities, symptom assessment, management and supportive counseling, and substance abuse education and intervention. All consumers discharged from Trenton Psychiatric Hospital are served by ICM for a minimum of 18 months. Second priority is given to consumers discharged from a short-term care facility. Limited community referrals are accepted. This program is provided in collaboration with AAMH.

Welfare to Work Mental Health Initiative

Case Management services are provided to individuals who are considered employable but whose mental health issues may be a barrier to employment. This is a collaborative project with Catholic Charities who provides supported employment for clients enrolled in this project.

Assists persons with mental illness to choose, find, get and keep their own place to live, through continuous support services and through the development of resources for safe, comfortable and affordable housing. Provides linkage to housing subsidies and vocational services, assessment, continuing support with daily living and interpersonal skills, short-term crisis stabilization and permanency planning. On My Own is a collaborative program of Catholic Charities, Greater Trenton Behavioral HealthCare and the Association for the Advancement of Mental Health.

Housing Search Services

Recruits landlords with private rooms to rent and boarding homes and rooming home operators who can provide safe and affordable housing for adults with a mental illness. Provides a matching service for consumers and landlords, with short-term support focused on resolving housing issues. The Housing Search Service is collaboration with Catholic Charities whose focus is on apartments.

Supportive Housing Services

Assists adults with a serious and persistent mental illness, who are enrolled in a GTBHC program, in finding and maintaining an apartment. Eligible individuals are homeless, at imminent risk of homelessness, or living in transitional housing programs. Services include identifying apartments, assistance in obtaining subsidies and ongoing support services related to landlord/neighbor relationships, skill development, linkage to services, illness management and recovery services and other supports enabling consumers to maintain housing.

Corrections Case Management Project

Provides case management services for offenders with mental illness who are being discharged from the Mercer County Corrections Center or a NJ State Prison with the intent of avoiding further incarceration or rehospitalization. Project also convenes countywide workgroup of corrections and mental health organizations focused on improving services for persons with a mental illness who come into contact with the justice system.

Crisis Diversion

The Crisis Diversion Program provides intensive case management, mini-partial services, and respite housing for clients who might be hospitalized in the near future without this acute intervention. Ten clients receive intensive services for one month and follow-up services for an additional month. Clients who receive this service are jointly chosen by the Screening Center and Crisis Diversion Program.
