Assists adults hospitalized at Trenton Psychiatric Hospital who need intensive support to reside independently. Services include identifying apartments, and ongoing support related to landlord/neighbor relationships, employment, linkage to services, illness management and recovery, and other services to help consumers maintain housing. Rental subsidies are provided. Support is provided 7 days per week, with 24 hour on-call availability.
