Assists adults with a serious and persistent mental illness, in finding and maintaining an apartment. Eligible individuals are homeless, at imminent risk of homelessness, or living in transitional housing programs. Services include identifying apartments, assistance in obtaining subsidies and ongoing support services related to landlord/neighbor relationships, employment, skill development, linkage to services, illness management and recovery services and other supports enabling consumers to maintain housing. Support is available 7 days per week, with 24 hour on-call availability.
