residents of Mercer County and their families. Services include
intensive day treatment; crisis-oriented outreach and in-home
services; outpatient services; and a variety of services for people
with special needs. Emergency on-call services are available 24
hours a day 7 days per week for those at the highest risk of crisis. |

The
Adolescent Partial Care program works with youth ages 12-17 who have
emotional/behavioral problems, as well as with their families.
Treatment modalities include group and individual services, as well
as family therapy. The program provides up to 5 hours of group-based
services daily. Other services include case management, skill
development in activities of daily living, relationships, anger
management and other essential skills, psychiatric evaluations and
medication monitoring, and transportation. Hours: M-F 11:30-6:30

Three
teams of case managers provide in-vivo and hospital based support
for consumers who have serious and persistent mental illnesses.
Services include assessment, psychoeducation and support, crisis
intervention, systems advocacy. Direct assistance in obtaining basic
necessities such as housing and entitlements, assistance with daily
living activities, symptom assessment, management and supportive
counseling, and substance abuse education and intervention. All
consumers discharged from Trenton Psychiatric Hospital are served by
ICM for 12 months. Second priority is given to consumers discharged
from a short-term care facility. Community referrals are also
accepted. This program is provided in collaboration with AAMH.
Hours: M-F 9-5. Early AM, evening and weekend hours as needed. 24

are made to the homeless drop-in Center at the Salvation Army,
Trenton Area Soup Kitchen, Rescue Mission, Homefront’s Family

engage persons who are homeless and have a mental illness. |

The Crisis Diversion
Program provides intensive case management, mini-partial services,
and respite housing for clients who might be hospitalized in the
near future without this acute intervention. Ten clients receive
intensive services for one month and follow-up services for an
additional month. Clients who receive this service are jointly
chosen by the Capital Health System Screening Center and Crisis
Diversion Program. |

Corrections Program provides case management services to assist
individuals with mental illness who are being discharged from the
Mercer County Corrections Center or a NJ State Prison and to divert
persons with mental illness who may be charged with a misdemeanor

Senior Well Being Program offers a series of interactive
workshops/presentations and supports groups to 1) adults ages 60 and
older and 2) caregivers on a variety of topics related to mental
health and wellness. The program also provides short-term
assessment, consultation and referrals for older adults or
caregivers who may need to access mental health or other resources.
An annual conference is also sponsored for Caregivers, and a
newsletter “Take Care” is published several times per year. Hours:

integrated into the program for individuals with a co-occurring

The Supportive
Housing Program assists adults with a serious and persistent mental
illness, in finding and maintaining an apartment. Eligible
individuals have a serious mental illness, are homeless, at imminent
risk of homelessness, or living in transitional housing programs.
Services include identifying apartments, assistance in obtaining
subsidies and ongoing support services related to landlord/neighbor
relationships, employment, skill development, linkage to services,
illness management and recovery services and other supports enabling

Housing
First is a program that provides housing to chronically homeless
individuals and families, There is no requirement for tenants to
participate in services, however support is offered in many areas
including landlord/neighbor relationships, employment, linkage to
services, illness management and recovery services, etc. Rental
subsidies are provided. Housing First is a demonstration project
carried out with support from the Mercer Alliance to End
Homelessness, DMHAS, DCA, the United Way, City of Trenton, County of
Mercer, NJ Housing Mortgage and Finance Agency, Corporation for
Supported Housing, Mercer County Board of Social Services, and
others. Support is provided 7 days per week, with 24 hour on-call

Intensive Outpatient Services provide a mix of skill building,
therapy groups, individual therapy, family therapy and
psychoeducation, medication management and case management.
Consumers eligible for the IOP have a serious and persistent mental
illness, which may include a co-occurring substance abuse disorder,
and a functional impairment in at least two life domains. They must
also be willing to participate in developing a Wellness Action
